It allows agencies to add optional notes, instructions, or requirements to support internal performance management, including administrative tracking like plan approval dates, organizational codes, or review comments. It captures agency-specific priorities or documentation beyond the standard form.
To access this page, from an open senior professional plan, click Agency Use from the navigation menu. The user may enter the notes into the text box. Click Save when done.
