The Employee and Plan Information Page allow senior professionals, rating officials, reviewing officials, administrators, and other users to review and edit required and optional fields related to the senior professional's performance plan. This information pre-populates the Consultation section of the performance plan.

Employee & Plan Information
The following fields are displayed under the Employee and Plan Information:
- Senior Professional Name
- Organization
- Pay Plan
- Rating Official
- Reviewing Official:
- Plan Start Date and End Date
- Title (Optional)
- Appointment Type
- Plan Purpose
- eOPF checkbox
Status
The plan status, at the top right corner, allows users to describe the state in which the plan is currently residing. In USA Performance plan statuses include the following: Active, On Hold, and Closed. For instructions, see: Update Plan Status
Appraisal Period
The appraisal period, also known as the Period of Performance, displays the time-period established by an appraisal system during which an employee's performance will be reviewed (5 CFR 430.203). The appraisal period may differ from the rating cycle if an employee starts or ends a plan outside of the rating cycle determined by agency policy.
Navigation Menu
The performance plan displays a navigation menu of the four main sections of the plans: Consultation, Progress Review, Rating, and Agency Use. Within each section, the user can view the critical elements, requirements, weights, applicable narratives according to your user role, and the Review and Sign page.
